My father is 57 and has diabetes. His weight is around 64 kg at 5’11”, which seems low. We want to make sure his diabetes is well controlled and check if there’s any underlying reason for his low weight.

64kg is not too low for the age. After 50 years usually there will be sarcopenia especially in Diabetes person.
To see weather Diabetes is well controlled need to check for RBS and HBA1C.
