Low best milk supply
Hi - My son is 6th week old. He is not able to latch properly due to flat nipple .. so my wife is using nipple shield make to latch , but she is not getting enough milk to feed baby .. when she pump with breast feeding baby for every 4 hours, she is getting 3OZ milk .. and if she breastfeed and pump immediately she is getting 1oz .. Baby was about 3kg when he born and at his one month checkup he is not gained much weight and he was just 3.4 kg on one month checkup.. 1. How much baby needs to drink at 6th week ? 2. My wife is started taking Lactare tablets from last 3 days (twice a day) but we did not see much improvement in milk flow.. How long Lactare will take start showing effect on milk supply? 3. Can she take galact granulas along with Lactare to see if there will any improvement? Since milk is supply is not great we are started feeding formula 3 times a day ..

Adequate feed is signified by ability to sleep 2 to 3 hrs after feed , Urine out put 6 to 8 times a day , adequate weight gain ( about 150 gms per week in the first 3 months of life ) 2. The effects starts in a few days 3. Yes she can take Milk supply is largely dependant upon complete emptying of rhe breast after a feed which is not as efficent with a breast pump as it is through natural emptying by  a baby's suck . Once you start top milk feeds the gaps between breast feeds become even more , the lacating hormones get lazy and milk supply further reduces . I would suggest that if you are giving formula give it over and above the breast feed rather than completely replacing the breast feed so that too long gaps between breast feeds are not there .This  will keep the hormones stimulated

Child should gain around 300 to 400gm in 2 weeks. For flat nipple, do nipple syringing before each feed (not nipple shield or any medicine). If child is passing urine 6 to 8 times in 24 hours with Breastfeeding then no need of formula milk Anyone of lactare or lactogenic granules is enough but it is for milk supply. If milk supply is low it is due to expressed milk and not breastfeeding. So continue your efforts for breastfeeding after nipple syringing for better milk supply.
