Hello Doctors, I am currently 33w+4  pregnant with high chance of pre term delivery. I was diagnosed with an insufficient cervix in 26w and funnelling which is now only 0.5cm left. I have an Arabin pessary. Amniotic sac is bulging through the funnelling .I had a small leak in 27w which is stopped after that.Cervix is dilated a bit ,doctors are not sure how much as they don't want to do many TVS due to fear of infection. I am on complete bed rest at home.Few days earlier as I was constipated some mucus plug was coming out during bowel movement due to a bit of straining now no mucus plugs are coming out. how do I know which point I should go to the hospital?  Will it be a rupture again? Can it be a silent dilation without pain as doctors appointments are 1-2 weeks away and no way to check cervix dilating in between.Can the labor start suddenly and fast which can rip off the cervix due to the pessary.Do I get time to go to the hospital? no mucus plug means the cervix opened a lot?

You should be in the hospital till you have a safe delivery. The membranes can rupture any moment and it may be too late to reach the hospital.

If you have contractions or a gush of thin watery discharge You will have to rush to the hospital If this is your first pregnancy once labour  sets and n u  will take time to deliver
