Hello, I am Dr. Romain Rajan, General Physician. Your case has been allotted to me.
I understand your concern. You recently had jaundice and now your reports also show high blood sugar. Both liver and sugar require careful management so that the recovery remains smooth and stable.
1. Advices
Eat simple home cooked food. Avoid fried, spicy and oily dishes completely till the liver enzymes become normal.
Avoid sweets, fruit juices, bakery items and cold drinks. Take three main meals and two light snacks daily at fixed times.
Walk daily for thirty minutes at a comfortable pace. Keep yourself hydrated with plenty of water and soups.
2. What to Monitor
Fasting and post meal sugar once every week for the next four weeks.
Repeat liver function test after one month to see if the SGPT and GGT values are settling down.
Note any yellowish eyes, itching, nausea or unusual weakness.
3. Management
Your fasting sugar is 204 mg per dl which means diabetes or poor sugar control. Liver enzymes SGPT 115 and GGT 446 are still high showing that the liver is recovering from recent jaundice or mild fatty changes. Bilirubin is almost normal which is a good sign.
You may start
Metformin 500 mg after lunch for five days, then continue twice daily after breakfast and dinner.
Liv 52 DS tablet twice daily after food for fifteen days to support liver function.
Continue light diet, avoid alcohol and unnecessary pain medicines.
If the sugar remains high or any weakness increases, a diabetic profile including HbA1c and lipid test will be required.
4. Diet Plan for Regular Use
Time Diet Plan
Early Morning One glass warm water with few soaked almonds (peeled)
Breakfast Oats or daliya or poha made in minimal oil with vegetables. One boiled egg or one glass toned milk without sugar
Mid Morning One fruit such as apple or guava or papaya. Avoid banana, mango and grapes
Lunch Two phulkas or one bowl brown rice with dal, one bowl vegetable, curd and salad
Evening Snack Green tea or black tea without sugar with roasted chana or murmura
Dinner Two phulkas with light sabzi and clear soup. Avoid rice at night
Before Bed Half cup toned milk without sugar if hungry
5. When to Consult a Doctor Immediately
If yellowing of eyes or skin returns.
If sugar rises above 300 mg per dl despite medicines.
If there is vomiting, severe fatigue or loss of appetite.
Every patientâs situation is unique. For accurate adjustment of medicines and long term sugar and liver control, it is important to discuss your history and response to medicines in detail.
