The chronic nature of these symptoms—specifically the testicular radiation—strongly points toward Inguinal Nerve Entrapment (involving the ilioinguinal or genitofemoral nerves). While the MRI shows scarring in the adductor muscles, this fibrosis often acts as a mechanical anchor that "tethers" the nerves passing through or near the inguinal canal, preventing normal nerve gliding during movement. This explains why the pain persists even after the initial inflammatory phase has passed. No you do not have to live with it
Next Steps
Consult a specialist ( physiotherapist) to perform clinical neurodynamic testing for the ilioinguinal and genitofemoral nerves to confirm entrapment.
Health Tips
Avoid aggressive "static stretching" of the groin, which can further irritate an entrapped nerve

Looking at your age and work history, groin pain and adductor scarring is suggestive of chronic adductor longus muscle strain/tendinopathy with post injury fibrosis. Referred pain in testicle suggest inguinal nerve involvement. Such condition can be treated with physiotherapy rehab focusing on eccentric muscles strengthening, core stabilization, soft tissue mobilization, pelvic control and load management. You may visit our clinic for further evaluation and treatment planning.
Next Steps
Structured physiotherapy rehab will be beneficial and has good prognosis, most patients improve in 2-4 months.
Health Tips
Avoid heavy lifting, wide leg weight bearing and sudden twisting temporarily until pain resolves.

For chronic adductor-related groin pain, physiotherapy should focus on controlled healing and nerve calming rather than aggressive stretching. The PT should start with pain-free adductor isometric exercises, gentle core and pelvic stabilization, and diaphragmatic breathing to reduce nerve irritation, then gradually progress to slow, controlled adductor and hip strengthening while addressing scar tightness with soft-tissue release. Sudden movements, deep squats, wide steps, twisting, running, or heavy lifting should be avoided in the early phase, and activities must be increased gradually based on pain response.
Next Steps
Use supportive underwear, avoid long periods of standing or awkward bending, apply heat before exercise and ice after if sore, and stop any exercise that causes sharp pain or increased testicular sensation. Consistency is key—do exercises daily, not aggressively—and expect gradual improvement over weeks rather than days.

Based on your history and MRI findings, you do not have to live with this condition permanently. There are effective treatment options available. Chronic groin pain with persistent testicular/groin sensation and MRI evidence of adductor muscle scarring is commonly seen in conditions such as chronic adductor tendinopathy, sportsman’s groin, or nerve-related irritation. Your previous work involving heavy physical activity may have contributed to this condition. The injection you received in 2022 likely reduced inflammation, which is why pain improved. However, muscle scarring and nerve sensitivity can cause ongoing discomfort or abnormal sensation. Treatment options include: • Targeted physiotherapy, focusing on: • Adductor muscle rehabilitation • Core and pelvic stability • Hip mobility and progressive strengthening • Manual therapy / myofascial release to address scar tissue • Nerve desensitization techniques if nerve involvement is present • Activity modification to avoid further strain • In persistent cases, image-guided injections or advanced therapies may be considered after evaluation Surgery is usually not required, unless a hernia or structural problem is identified.
Next Steps
1. Consult a qualified musculoskeletal physiotherapist 2. Begin a structured rehabilitation program for at least 6–8 weeks 3. Avoid heavy lifting, sudden twisting, or overloading activities 4. Follow up if symptoms do not improve with rehabilitation
Health Tips
Avoid depending only on injections for long-term relief. Proper physiotherapy and guided rehabilitation are essential for recovery. For professional assessment and home-based physiotherapy support, you may visit:
