Immunization proof for US student
Hi,
I am born on 30th October 1991 at Moga,Punjab and doesn't have vaccine records with me now. For US University ,I require proof of vaccines of MMR, Hepatitis B and TB. I have got all vaccines given in 90s including hepatitis B.
Please let me know if any doctor can get this certified.
