I need a medical certificate for 3 month

Hi, I am suffering from Ankylosing Spondylitis past 1 year. Due to covid situation, I had to take 3 months leave due to flare ups and other complications. I did not visit any hospital due to covid risks. I was continuing the medicines prescribed to me but I don't have any leave certificate as such from any doctor. I have all lab test reports and MRI scans with me. Can someone give me a medical certificate? How do I apply? Thanks, Debanjana
