Hypothyroidism
Hello doctor. My 8 year old daughter was very slim till last year suddenly gained weight but the weight is consistent. She is 115 cm and 28 kgs. She is physically active and very good in school. She rarely eats junk n sweets. We consulted paediatrician and he advised blood tests. Her t3, t4 are very low and tsh>496. Vit d is 6 B12- 165 Cholesterol total- 296 Hba1c -6 Doctor has started thyroxine 25mcg and multivitamins. Is this reversible?? Is the cholesterol due to hypothyroidism? When will the results be seen? Please suggest.

Tsh is very deranged. Most of the abnormalities can be attributed to thyroid dysfunction. Repeat the test after 1m. Changes will definitely be seen but would not be completely normal.

Hypothyroidism is due to many causes. Since TSH is so high, get your child tested for antibodies ( ANTI TPO, anti TSH receptor antibodies). Some cases are reversible but some require lifelong support. Also get an echo done along with a thyroid ultrasound. Yes cholesterol is because of hypothyroidism, causes deranged lipid profile. Repeat TSH after 4 weeks. Dose might need titration. You need to follow up with an endocrinologist for further management.
