The description is slightly vague as to ascertain whether the damage has been done to the gums or the tooth or both. Please visit a dentist as soon as possible to evaluate the exact condition of the tooth. The right treatment can be instituted only after a diagnosis has been made. The treatment can vary from repair of the damage to the tooth structure or or in worst case, even extraction of the tooth. An x-ray might also be required to aid in the clinical diagnosis. Regards.

What i have judged from chief complaint is that most probably you have fractured your tooth while opening the bottle cap.it requires immidiate attention from a dentist as you said its becoming shaky.visit your nearby doctor to get it fixed before you lose your tooth
Next Steps
immidiate visit to the doctor (dentist)
