Hello
I have been experiencing low libido, mood swings and fatigue during the day for the past few months. I do my workouts regularly and am physically quite active. Recently I also realized that I am not able to gain muscle easily and fat loss is a challenge
I got all my blood works done which seems normal. I also got a hormonal profile done
Testosterone total is 3.7 ng/ml
SHBG is 32.50 nmol/l
DHEAS is 78.6 ug/dl
Vitamin D and B12 is normal
I am not sure if these hormonal levels are causing the concerns and request guidance

Possible causes include hormonal imbalance, low testosterone or low free testosterone, vitamin D or B12 deficiency, stress, anxiety, poor sleep, overtraining or inadequate recovery, thyroid issues, depression or mental stress, poor diet/protein-calorie imbalance, insulin resistance/obesity, and hormonal imbalance such as elevated prolactin levels.
You should come to our clinic. We will examine you properly and repeat the necessary blood investigations to find the exact cause and then advise treatment accordingly.

Namastey
Your total testosterone of 3.7 ng/ml (â370 ng/dL) falls in the low-normal range, but symptoms matter more than numbers alone. Some individuals experience features of Hypogonadism even within ânormalâ lab limits, especially if free (bioavailable) testosterone is low. Your SHBG is within range, but DHEAS appears relatively on the lower side for your age, which may contribute to fatigue and reduced vitality.
These symptoms can arise due to a combination of factors:
Suboptimal androgen activity (even if total testosterone looks normal)
Stress, poor recovery, or sleep disturbance affecting hormonal rhythm
Metabolic factors or overtraining
Early functional hormonal imbalance rather than a fixed disease
In many such cases, the issue is functional and reversible, rather than a permanent endocrine disorder. Ayurveda correlates this with Dhatu kshaya (tissue depletion) and imbalance of Vata, affecting energy, mood, and reproductive strength (Shukra dhatu).
Next Steps
A deeper evaluation is advisable before concluding. You may consider: Free testosterone and LH/FSH levels Thyroid profile (if not already done) Cortisol (to assess stress axis) Rather than starting hormones directly, the focus should be on root-cause correction and hormonal optimization. Based on your symptoms, a personalized treatment planâcombining lifestyle correction, nutritional support, and targeted Ayurvedic therapyâcan significantly improve your condition. Health Tips
nsure quality sleep (7â8 hours), as testosterone and recovery hormones are highly dependent on sleep cycles. Avoid late-night screen exposure and maintain a consistent routine. Nutrition plays a key roleâinclude healthy fats (ghee, nuts, seeds), good protein intake, and micronutrients like zinc and magnesium. Avoid excessive calorie restriction if you are trying to lose fat, as it can further suppress hormones. Also, avoid overtrainingâbalance intense workouts with adequate recovery, as excessive strain can lower hormonal efficiency. Thank You
