Sir, aapke symptoms zyada fissure (choti si tear) ke lag rahe hain, piles se zyada. Stool ke baad burning pain aur fresh blood aana fissure me common hota hai. Abhi starting stage me hai to surgery ki zarurat nahi hoti. Sabse important hai stool soft rakhna. Iske liye fibre diet badhaiye (salad, fruits), pani 3–4 litre lijiye, aur ek mild stool softener raat ko le sakte hain. Local relief ke liye din me 2–3 baar garam pani me 10–15 min baithna (sitz bath) kaafi help karta hai. Saath me lignocaine + nifedipine/diltiazem wali ointment local apply kar sakte hain pain aur spasm ke liye. Spicy food, constipation aur zyada strain avoid kariye. Aksar 2–3 weeks me theek ho jata hai. Agar pain/blooding continue rahe ya lump badhe to ek baar surgeon ko dikha dijiye.

Severe burning pain that lasts for hours after passing stool, accompanied by bright red blood, is highly characteristic of an Anal Fissure (a small tear in the lining of the anus). While you have identified a 'skin tag,' this is often a 'Sentinel Pile,' which is a secondary skin growth that forms at the edge of a chronic fissure. This pain-bleeding-tag triad suggests the condition has progressed beyond the initial stage and requires active medical management to prevent the need for surgery."
Next Steps
Sitz Bath: This is the most effective immediate relief. Sit in a tub of warm water for 10–15 minutes, 3 times a day. This relaxes the anal sphincter muscle and reduces the burning pain. Fiber & Stool Softeners: , you must keep your stools very soft. Increase water intake and use a natural fiber supplement (like Isabgol) to prevent further tearing. Topical Ointments: You likely need a prescribed ointment containing a local anesthetic (to numb the pain) and a calcium channel blocker or nitrate (to improve blood flow and heal the tear). Dietary Discipline: Avoid spicy foods and caffeine, which can irritate the digestive tract and make bowel movements more painful. Mandatory Surgical Consultation: Chronic fissures with sentinel tags often require specialized care. A formal consultation with a General Surgeon or Proctologist is mandatory. A physical examination (digital rectal exam) is necessary to confirm the diagnosis and rule out internal hemorrhoids or fistulas.
Health Tips
the 'hours of pain' suggest your internal muscle is in spasm, which prevents the tear from healing. Please book a detailed consultation so we can prescribe the specific healing creams and muscle relaxants needed to close the fissure before it requires surgical intervention."

Not piles.its anal fissure due to chronic constipation and hard stool .due to hard stool its rupture the anal wall laterally and that's why you have bleeding also during defecation. 1.sitz bath -3 times a day ,15 minute each time. 2.isabgol 1 tbsp .with curd and plenty of water. 3.keep diet more fibrous. 4.anovate gel for healing. Consult me for further management and guidance.
