Have you consulted Hematologist? Any abnormal bleeding disorder? If it doesn’t respond to harmone treatment, Mirena is a kind of harmone treatment, it will not respond to it also. But 95% patients respond to either of allopathy treatment you have mentioned.
Mirena can be good option in selected patients relief may be expected in 8in 10 cases depending on detailed case scenario.however if your wife is suitable candidate for mirena or any other modality of treatment and need for endometrial biopsy will need evaluation .
If hormones have not worked, next best is to do a D and C , with mirena insertion. Mirena produces progesterone locally only in the uterus, so the side effects are less when compared to oral hormones. The mirena can stay in place for 5 years, success rate is about 95 percent by 1 year .
If the cavity of the uterus is not distorted by firbroids or polyp or if the size of the uterus is not too big , mirena works well. Your gynaecologist will look into all these before suggesting mirena
