Hi, I’m 27 years old female and currently weigh 100 kg and height is 5'5". I have been working out regularly for 6 days a week and eating normal home cooked food, but I’m not seeing any meaningful weight loss. I also have hypothyroidism — my TSH was around 11 six months ago. I do feel low energy and fatigue quite often despite trying to stay active. I’ve also noticed signs like dark pigmentation around my neck and under my eyes, which I’ve read could be related to insulin resistance. I feel like despite consistent efforts, my body is not responding, and I’m getting physically tired. I want to understand if there’s an underlying metabolic issue. Could you please guide me on what tests I should do and whether medical treatment options like GLP-1 medications would be appropriate in my case?

As you told you are having dark pigmentation around neck ( that is acantosis nigricens ). These are signs of insulin resistance so there is a risk of Diabetes so first get your FPG,  PPBG and HbA1c done. You should also get lipid profile and LFT done. You are having insulin resistance because of obesity and  insulin resistance causes diabetes, high cholesterol, fatty liver, PCOS, and hypertension as well. if you lose weight,  your body insulin resistance will also decrease. Obesity treatment starts from lifestyle modifications. So take 1200 kcal diet per day and do atleast 1hour physical activity. Try this method first because once you stop GLP1  RAs, you will regain the same weight.

Hi, You can start GLP 1,but before that evaluate underlying cause.It could be any thing pcos, hypothyroidism or any metabolic disorder.go thoroughly blood check up includes diabetic and LFT profile.you are countered with fatigue, weakness do vitamin profile either. Thanks
