I have been suffering from GERD for approximately 10 years. My symptoms have been persistent and significantly affect my daily life.
I experience severe heartburn and a constant sensation of breathlessness, even with minimal physical activity such as walking or slight body movements. The breathing difficulty is one of my most concerning symptoms. I did endoscopy 4 times so far , did 24h ph test and esophageal Manometry as well my demeester score is 17 and Acid exposure time is 4.2% and Manometry is normal
I have tried multiple treatments over the years, including (PPIs, PCABs), and low-dose amitriptyline, but none have provided lasting or significant relief.
Despite ongoing treatment, my symptoms continue to worsen or remain unchanged, and my quality of life is severely impacted.
I would like to understand whether surgical options such as anti-reflux surgery could be effective in my case, and what evaluations or tests would be required to determine if I am a suitable candidate.

do
-You are a potential candidate for anti-reflux surgery (e.g., fundoplication) when:
-Persistent symptoms despite optimal PPI therapy
-Objective reflux proven
-Normal motility
-Before surgery, required evaluation:
-Repeat/confirm 24h pH impedance
-High-resolution manometry
-Endoscopy to rule out complications
-Symptom correlation with reflux episodes
-Discuss with GI surgeon / upper GI specialist for procedures like:
-Laparoscopic Nissen fundoplication
-Partial fundoplication (if needed based on motility)
Health Tips
-Surgery helps typical symptoms (heartburn/regurgitation) more than atypical (breathlessness)
-Continue lifestyle: weight control, head-end elevation, avoid late meals
-Avoid unnecessary escalation of medications if surgery planned

MENTIONED ALL ABOUT TEST NO OF TIMES BUT WHAT ABOUT ACTION??? WHEN U GO FOR ACTION ALTHOUGH SUFFERING IS IMENCE.MEANS SURGERY NOW ITS EMERGENCY. GO FOR SURGERY.
Since your Manometry is normal, your esophagus has the muscle strength to handle a wrap without a risk of permanent swallowing issues. Surgery is more effective than medication for respiratory symptoms ,which PPIs cannot stop. take a Surgical Consultation to confirm if surgery will fix the breathlessness, you need tests like Barium Swallow, Gastric Emptying Study.
Next Steps
Use a Wedge Pillow to keep gravity working for you at night. Do diaphragmatic breathing.Practicing deep belly breathing can strengthen the diaphragm and may reduce the sensation of breathlessness. Eat 5â6 very small meals instead of 3 large ones to prevent stomach distension, which often triggers the breathlessness you feel during movement
Since you are taking treatment for the last 10yrs without relieving the symptoms, you can opt for surgery that is called Fundoplication. The surgery is done under general anaesthesia and done by a surgical gastroenterologist. For the anaesthesia part, pre anaesthetic evaluation should be done that includes blood work , ECG , CHEST X-Ray and physical examination. Once you are declared fit by Anaesthesiologist and all required investigations are done by the surgeon, the surgery can be performed
