I had phymosis and had circumcision and frenuloplasty 2 years back. Iam having frequent nightfalls(everyother day) post circumcision. When erect, the underside of the shaft where frenulum is done is hypersensitive and I think it is triggering nightfall due to friction with clothes during sleep when erections happen in REM sleep.
It would be helpful if any doctor can help me with a solution? Frequent nightfalls are severely affecting my quality of life with fatigue, sleeplessness and stress. A photo of the exact spot of hypersensitivity is attached for reference. Pardon me if photo looks objectionable.

What you are describing can happen in some men after Circumcision and Frenuloplasty, especially if the area on the underside of the penis remains unusually sensitive during erections. The frenular region contains many nerve endings, and after surgery some individuals may develop persistent hypersensitivity or heightened sensory awareness, which can make friction from clothing or nocturnal erections feel exaggerated. This may contribute to increased arousal during sleep and frequent Nocturnal emission episodes.
However, it is also important to understand that anxiety, hyperfocus on genital sensations, disturbed sleep, and constant anticipation of nightfall can further intensify the cycle. Frequent nightfalls themselves are not physically dangerous, but when they become repetitive and mentally distressing, they can affect sleep quality, energy, and emotional well-being. Since your symptoms have been continuing for a long time after surgery and are affecting your quality of life, this deserves a careful and personalized evaluation rather than ignoring it or relying on temporary measures.
Next Steps
Because your symptoms are persistent and specifically linked to a post-surgical sensitive area, a proper consultation would genuinely help in identifying whether the issue is mainly sensory hypersensitivity, scar-related sensitivity, pelvic floor tension, or a stress-related amplification pattern. A focused discussion and examination can often provide reassurance and a structured recovery plan. Health Tips
Maintain regular sleep timings and avoid excessive sexual stimulation or overthinking about erections before bedtime. Practice relaxation techniques such as deep breathing or meditation to calm the nervous system. Keep the genital area dry and comfortable, and avoid harsh soaps or irritation over the sensitive scar area. Most importantly, try not to associate every nightfall with physical weakness or damageâstress and fear themselves can worsen the cycle. With proper guidance and a systematic approach, many men experience significant improvement in sensitivity and overall comfort over time. Thank you
