Food Coming Back to Mouth
Hello Doctors, I am a 28-year-old male. Since childhood, I haven’t chewed food properly, which has caused food to come back into my mouth. Now, I have asthma and small airway disease. I am currently taking pantoprazole, which has reduced the issue slightly, but food still comes back to my mouth. What could be the reason for this, and what is the best solution? Should I undergo any tests or change my medication? Please advise.

Hello,  Thank you for sharing your concerns. Based on your description, the issue of food coming back into your mouth (likely regurgitation) could be related to several underlying conditions, such as:  - Gastroesophageal Reflux Disease (GERD): Chronic acid reflux can cause regurgitation, especially if untreated.  - Esophageal Motility Disorders: Improper chewing combined with weak esophageal muscles may contribute to this problem.  - Asthma and Small Airway Disease: These can sometimes exacerbate or coexist with GERD due to shared mechanisms like increased abdominal pressure.  Pantoprazole helps reduce stomach acid, but if symptoms persist, further evaluation is needed. I recommend:  1. Diagnostic Tests     - Upper Endoscopy To check for esophageal damage or strictures.     - Esophageal Manometry: To assess muscle function in your esophagus.     - pH Monitoring: To measure acid levels over 24 hours.  2. Lifestyle Modifications:     - Eat smaller, slower meals and chew thoroughly.     - Avoid lying down immediately after eating.  3. Medication Adjustmen:Depending on test results, a combination therapy (e.g., prokinetics) may be more effective.  Your asthma and airway disease should also be evaluated in context, as reflux can worsen respiratory symptoms.  For a detailed assessment and personalized treatment plan, I strongly advise scheduling a consultation.
