Fever not getting down after giving pcm
Dear Doctor, my son is 9 years old, weight 24 kg, he got fever 102 suddenly today , given paracetamol but fever reduce for 30 min than came back. Please help what should I do.

Looks like viral fever. Mostly a second dose of paracetamol can help. During first 3-4 days only paracetamol and cold water sponging. Beyond 4th day fever requires investigation. 9 years old child. No need to worry. Increase water and other fluids intake and give soft diet. Gradually fever should come down in case of viral fever. But may require investigation depending on symptoms.

Paracetamol helps reduce fever temporarily but does not treat the underlying cause. It is common for fever to rise again once the effect of the medicine wears off. For a 24 kg child, ensure that the correct dose of paracetamol is being given (15 mg/kg/dose every 4–6 hours as needed, not exceeding the recommended daily dose). Encourage adequate fluids and light meals. Please monitor for associated symptoms such as cough, cold, sore throat, vomiting, diarrhea, headache, rash, burning during urination, or lethargy, as these may help identify the cause. If the fever persists beyond 48–72 hours, becomes very high, or your child develops breathing difficulty, excessive sleepiness, dehydration, seizures, or appears very unwell, seek medical evaluation promptly. At present, a single day of fever with temporary response to paracetamol is most commonly due to a viral infection and can be observed closely.

After 102 paracetamol won't work So Use Syrup Meftal-P give 8ml every 6th hourly

Sometimes paracetmol alone cannot control higher temperatures Can give syrup meftal-p 7.5ml every 8-12 hrly if temp> 102 Please book a consultation for proper evaluation
