Ethic/Legal Question/Diagnosis Issue
I have a question. First I’ll give you background: I am participating in a research study (paid) and because I “indicated that I induce vomiting and/or overuse laxatives” they are requesting that i get a consent form from my doctor to participate. The consent form must say that I am a healthy adult and I am not sickly. If I take this form to my doctor, will I be forced to talk about it? And if I talk about it- could this result in a permanent diagnosis of “bulimia” or any other form of eating

No it wont lead to a "permanent" diagnosis. Its upto you if you want to participate or not. The research people want to make sure that if you participate, you should fit the inclusion criteria.
Next Steps
You may visit a mental health specialist if you have habit of inducing vomitus.
