Esophegus carcinoma
I am attaching the investigation report of Biopsy of my mother in laws. Oncologist has advised for radiation and chemotherapy. Should I go with the advise or is there some other treatment option are still available before starting of chemotherapy and radiotherapy for treatment of esophegus carcinoma. Please suggest.

Its depend on site of oesophagus upper middle or lower if its upper go for chemo and radiation depends on patient GC if lower may be CTRT or surgery depends on other criteria so complete work up is required go for PET CT and send imaging for final decision

I am discussing possible scenarios as many information is not available in your post. You have attached biopsy report which says it is squamous cell carcinoma. If you could attach the CT report of thorax or PETCT whichever you have done, that will help me to understand the extent of disease or stage also. Treatment depends on if the disease is metastatic or localised. Considering the fact that your doctor has offered radiation therapy, the tumour may be localised and not spread anywhere else distant. Also considering the mention of squamous cell carcinoma, the location may mostly be in the upper or mid esophagus. 1. If it’s upper esophageal lesion, the treatment what you have mentioned chemo and radiation together at a radical dose will be the treatment. 2. If lesion is in lower esophagus and localised, then chemo and radiation will be at a lower dose and then surgery has to be planned for removal of tumour. 3. If it’s metastatic, treatment will be with chemo / systemic therapy only. If you could attach the scan reports and other reports, or do an online consult, we can discuss further.

Hi... In esophageal cancer number of factors will decide the treatment of choice like site of cancer( upper, middle , lower end) , stage of the disease ( assessed by either CECT thorax+abdomen or PETCT) and general condition of the patient like weight, other medical illness , is he taking orally full diet, how he is medically fit to take the treatment. Both scanning and patients tolerability to take the treatment decides what is the next course. Ideally in esophageal cancer except in upper one third all patients should undergo surgery. But surgery is done usually after giving lowdose chemoradiation...only small short segment esophageal cancers we can operate upfront without giving chemoradiation. Giving only chemoradiation for patient is enough if it's upper onethird esophageal Cancer or else surgery is definitely required.
