Effect Of Empagliflozin Or Nebivolol ?
My urine ACR level was 5.77 mg/g (Normal <30 mg/gm) on 18/05/2025 i.e just before starting Empagliflozin Previously it was 2.84 mg/g on 4/4/2025 Serum creatinine,potassium,urea & usg are all within normal limits Taking Empagliflozin 10 mg once a day  for blood sugar & Nebivolol 5 mg once a day for blood pressure since 20 days Recently noticing foamy & frothy urine Is it due to empagliflozin or Nebivolol ?

Foamy or frothy urine can be due to multiple causes. In this case, since your urine ACR is still within normal range (<30 mg/g) and other parameters like serum creatinine and USG are normal, it's less likely due to kidney damage. Empagliflozin increases glucose excretion in urine (glucosuria), which may sometimes cause foamy urine — this is usually harmless. Nebivolol is not known to cause foamy urine or affect urine composition in this way. However, if foamy urine persists or worsens, repeat urine ACR and consider a urine protein/creatinine ratio to rule out early kidney involvement. consult

These drugs generally are considered safe and are not associated with increasing U ACR as side effects. Although gliflozins are commonly linked to urinary tract infections which may transiently lead to frothy urine with burning sensation. Be rest assured as U ACR of 5.7 is still way below normal.

Hi Foamy frothy urine usually due to protinuria. You are looking at ACR which is Albumin but other proteins can also released in urine which can cause frothy urine ,for which you have e check PCR. Neither Empagliflozin or Nebivolol can cause frothy urine But empagliflozin can cause glycosuria which is release of Glucose in urine. Empagliflozin can improves the kidney function according to Many trials, you can refer Emperor trial. That's why your Urea creatinine is improving. Check Spot PCR ,24 hr Urine PCR for protinuria.
