Doubt about misscarry

Asked for Female, 26 Years

My last period on 22 sep 2017 and got UPT positive on Nov 06 . doctor scanned and said endometrial thick Ness is 18 mm rich but no sac. suddenly I got severe pain at right pelvic area with bleeding also decresed B-HCG levels .Now that level is 40 . What should I do now . Someone said u should do scan again for clearance .Is it necessary.misscarry is it.
