Diabetes sugar review dosage

Asked for Male, 27 Years

My father aged 60 years old CABG operated patient suffering from diabetes under insulin from past 9 years. He is having ureter stone frequently and recently operated. He is taking Human Actrapid 15-0-15 before 15 minutes of taking food. And Lantus 0-0-20(during bed time). His fasting is at 199. Hba1c is 8.9. Pls suggest some other insulin or increase the dosage quantity.
