Diabetes/Pleural effusion/TB medicines
My wife was detected with diabetes on 17 Nov'24, she started with diabetes medicines as HB1Ac was 11. On 19th Dec'24 after complaining, breathlessness it was found that her left lung collapsed due to plural effusion. Hospitalized for five days. Plural effusion checked, and results showed TB. TB medicine started 23rd Dec'24. A patch on the chest was seen in the CT scan and x-ray before she was discharged from the hospital. Today it's been four months and the patch in the left lung is still the same. The city report says. "There is relatively well defined soft tissue density mass like focus is seen projected over the left middle zone adjacent to the left hilum with central area of lucency representing central breakdown. The left hilum is seen through the lesion. The lesion is almost completely incased by the lung lucency except for part of its medial margin. The patient is well gained weight managing diabetes well and in control. Goes to work every day, gaining weight. Pls advise?

Ct scans will aid in our clinical diagnosis.. but ct scan alone will not provide us perfect diagnosis… if clinically her symptoms getting resolved compared to earlier and getting better ,and also diabetes under control,, continue tuberculosis treatment as per guidelines n recommended duration by your doctor… if she has any new symptoms or symptoms not getting resolved as earlier, requires further evaluation to check whether along with tuberculosis any other diagnosis we are missing … U can upload ct films n report as well for further detailing

I can advice 3.things 1.you can go for day care procedure called bronchoscopy if required later EBUS AND BIOPSY. 2. You can go for CECT THORAX to find out what kind of mass lesion or is it lung atelectasis (collapsed region of lung)/something else like cavity…. Do check renal function tests as she is a diabetic before going to CECT. 3.Uploading films and report will be helpful otherwise there are hundreds of diseases and differentials.

please share your ct films along with the report over a consult..other causes apart from tb must also be ruled out
