My father is on Istamet DXR - 500 but recently his blood sugars have peaked and hba1c is 7.
So doctor has suggested additional dosage of vildagliptin 50 in the night.
I wanted second opinion on this. If two different gliptins are not recommended, and if Istamet d alone is not sufficient then what are the options? Kindly suggest.

For HbA1c 7% on Istamet D-XR, adding Vilda (another DPP-4 inhibitor) is generally not recommended due to class duplication (sitagliptin vs vildagliptin), risking limited extra benefit and higher costs
Next Steps
Continue Istamet D-XR alone
HbA1c of 7% indicates moderately controlled type 2 diabetes, aligning with the upper target range for many adults per RSSDI guidelines (7-8%)

Our target is Hba1c 7. So do not worry. If Hba1c goes below 7 in elderly, low glucose can occur, which is danger. So 2 glyptin addition is not necessary. Pl consult your doctor.
