Hi. I understand your concern s. Yes it is mostly early signs of COPD . Better to stop smoking and also undergo a pulmonary function test which is needed for the diagnosis. Consult a pulmonologist for the same. Also if you need help to quit smoking the pulmonologist will do the needful.
For your age group, with chronic smoking history and CT chest showing airway trapping could be a sign of RBILD (Respiratory Bronchiolitis - Interstitial Lung Disease), specifically if CT also shows ground glass appearance or mosaic patterns.
Although early onset COPD is also possible, specially associated with a family history or Alfa 1 antitrypsin deficiency or extremely heavy smoking (as in your case)
Next Steps
A spirometry with reversibility can diagnose accurately.
Health Tips
COPD is usually progressive even after smoking cessation but RBILD have a very good outcome if patient quit smoking on time.
You may consult a Pulmonologist.
