For your age group, with chronic smoking history and CT chest showing airway trapping could be a sign of RBILD (Respiratory Bronchiolitis - Interstitial Lung Disease), specifically if CT also shows ground glass appearance or mosaic patterns. Although early onset COPD is also possible, specially associated with a family history or Alfa 1 antitrypsin deficiency or extremely heavy smoking (as in your case)
Next Steps
A spirometry with reversibility can diagnose accurately.
Health Tips
COPD is usually progressive even after smoking cessation but RBILD have a very good outcome if patient quit smoking on time. You may consult a Pulmonologist.
