Constant period for months

Asked for Female, 32 Years

For the past few months i've had a constant period...some days are light while other days are heavy. I always feel tired and drained and I dont know what's going on...there are heavy clots and is usually the worst over night. I am a trans man (Female to male) and have not been on testosterone since late 2015.
