In your report, protein +, few granular casts and bacteria are present. It indicates severe infection. 5 days antibiotics course will never clear it. My suggestion is to take homoeopathic treatment after 5 days course. So that, there will not be antibiotics resistance and you could prevent recurrency too.

Well,
It’s common that urine findings like granular casts and protein+ may still persist even after 2–3 days of treatment. These changes do not always normalize immediately with antibiotics like Amoxicillin and Clavulanate.
A 5-day course is usually sufficient for uncomplicated UTI, but:
Protein+ can take some time to resolve as inflammation settles
Granular casts may indicate mild renal tubular irritation and can persist temporarily..

Still uti can reoccur i saw ur reports
I suggest u to take homeopathic Medicine so the uti will not reoccur
