Can chronic anal fissure heal by themselves or mith medicine? Does these always require surgery? What not to eat?

Good afternoon, I am Krishna and I am a UK 🇬🇧 trained Gastro Intestinal and Bariatric Surgeon practicing in Hyderabad and Bangalore and currently moved back to UK. If you go by evidence there is not anything you cannot eat....all we advise is avoid constipation. Unfortunately chronic fissures are notorious to treat medically. However if you hesitant about surgery ( which is understandable) then we in UK (and its happening in India as well) do inject Botox and Hyaluronic acid. If you need any further help do revert back. Anal fissures are cuts in the anal canal that happen due to constipation, hard stools, tight muscle tone and other factors. Chronic anal fissures are long standing thickened cuts in the anal region that do not heal completely with medications like ointments and laxatives. Those fissures that have not healed inspite of medications will need surgery. Regarding food, you must have a healthy diet with lots of fibre. Avoid only the food items that you know give you constipation. Some of the food items that may cause constipation(not necessarily for everyone) are: Red meat Chanda dal Besan However, eating sufficient fibre-rich foods and drinking atleast 3-4lites of water everyday is recommended.
