Rest all is flavouring and other minor components which do not have a major role in maintaining oral hygiene, contrary to what the manufacturers advertise. Both the herbal and the chemical toothpaste provide the same benefit because it is the brushing technique which makes the difference in maintaining the oral hygiene. Again, contrary to what is advertised, even the herbal toothpaste contain the same chemicals. So practically speaking, they do have other herbal and ayurvedic medicines in them but they also contain those chemicals which other toothpaste contain which are synthetic only. You can check the composition of these toothpastes to understand that the herbal toothpaste are not technically "chemical free".

It will be recommended not to use things like bamboo sticks or Neem sticks or charcoal powder because they might have some advantages but the disadvantages are more. It is not actually correct to say that earlier people did not have dental problems, because if you go by the statistics, the dental problems had been equally prevalent even in free civilized era. The only difference was that the diet was more fibrous as compared to the soft and sticky diet that we have today, and also the fact that extraction was the only solution to dental pain, and people with even highly mobile teeth did not seek treatment if pain was not bothering them.
