Prefer sponging with lukewarm water, give enough water and other fluids, take paracetamol, roughly a 4 year old would require, 4ml(250 mg) three times a day.

As you say severe/high fever give only paracetamol 10mg/kg every 5hours better meet your paediatrician on line Vedic
