Chest muscle tightness, red skin.

This evening it felt like the left side of my chest(near the nipple are), felt like it's getting a little tight. I applied some ointment there and later there was some relief. Then sometime later, a red circular rash type showed up which felt a little itchy sometime. I have attached the picture, are are with white encircling is where the rash is, blue encircle denotes the area when i was feeling tightness. Also previously(a week ago) I had some hives formed on my hand and later they turned to small lesions. Please help me, m I going through any infection or systemic disease?
