Hi
Not just the black spots in his centrals, there are active caries on the adjacent teeth as well (note the chalky white areas near the gums). They will fall much later. He will benefit from restorations (fillings) as well as active fluoride therapy, and a complete caries risk assessment.
Next Steps
Take a pea sized amount of toothpaste and keep brushing his teeth twice daily. After brushing, once he spits the froth out, DO NOT RINSE with water. Let some toothpaste stay over the teeth. Non-ayurvedic toothpastes (colgate, kidodent, pediflor, pepsodent) contain fluoride which will help put calcium back inside his teeth.
You can visit my clinic in Greater Noida West, for professional fluoride application. I can guide you for further treatment, in person.
Health Tips
If he keeps swallowing the toothpaste, don’t use the fluoride toothpastes. Instead use Pigeon’s fluoride-free toothpaste.

Yes Visit a Pedodontist, he will need fillings. If on xrays, it shows that the infection has reached nerve, root canal would be done to preserve the tooth health.Kindly dont wait for them to fall as it might affect the permanent tooth that will erupt and can also alter its path of eruption.
Health Tips
make sure your kid brushes twice daily and rinses after every meal, avoid any sugary foods
Just wait for 6 months ,these teeth will get replaced by permanent teeth with passage of time, black spots are cavities but anyway these teeth are temporary
Hello.
Your baby’s central two incisors have cavities.
Considering his age, permanent central incisors will erupt soon.
Please visit a dentist for clinical evaluation to decide whether to go for a tooth filling or to wait if they are moving and permanent incisors are on their way.
