Go for iopa xray,to determine the extent of decay. Depending upon finding of xray we can decide what to do.

Hie.. we need to take an X-ray for confirmation since it seems to be severely decayed from the other end .. but since he is just 4 years old, we try to save the tooth with rct.. after investigation only ..we can rule out the exact.. if it gets extracted then do go for space maintainer.
Next Steps
Kindly consult and get an X-ray first

Visit a pediatric dentist. After examination which will include a detailed history, clinical examination and xray we can decide what treatment would be best for the child.
