Black patches on tongue & buccal mucosa
Hello Doctors, I have noticed black patches on my tongue. I first observed them about 2 weeks ago, but I am not sure when exactly they developed. There is no pain, discomfort, or change in taste, but I am concerned if this could be something serious. I have attached a photo for your reference. Kindly advise if this requires further investigation or treatment. P.S: I am currently undergoing homeopathic treatment for hidradenitis suppurativa.

1. Physiological oral pigmentation – common in darker skin individuals. 2. Smoker’s melanosis / tobacco-related pigmentation – if history of smoking/tobacco. 3. Medication-induced pigmentation – antimalarials, minocycline, heavy metals. 4. Oral lichen planus / post-inflammatory pigmentation – usually with lacy/patchy appearance. 5. Oral candidiasis (chronic hyperpigmented) – if immune suppression. 6. Systemic causes – Addison’s disease, Peutz–Jeghers syndrome. 7. Oral melanoma – rare but important to exclude (look for asymmetry, rapid growth, bleeding, irregular borders). --- ✅ Advice to Patient The patches do not look immediately dangerous (flat, non-ulcerated, non-painful). However, since oral mucosal pigmentation can rarely be due to premalignant or malignant lesions, evaluation is needed. Next steps: 1. Clinical examination by dentist or oral medicine specialist. 2. History: smoking, alcohol, medications, systemic illnesses. 3. If uncertain → biopsy of suspicious patch. --- 🔑 Interim Advice Maintain strict oral hygiene (brushing, tongue cleaning, antiseptic mouth rinse). Avoid smoking, tobacco, or alcohol (if applicable). Monitor for changes in size, color, surface, pain, or bleeding → report immediately. --- 🚨 Red-Flag Features (require urgent specialist review) Rapid increase in size. Ulceration / bleeding. Pain or burning. Irregular or raised lesion. --- 👨‍⚕️ Final Consultation Note for Patient > The black patches on your tongue appear benign but could be due to pigmentation from various causes, including smoking, drugs, or normal variations. Rarely, oral mucosal pigmentation can indicate precancerous or cancerous conditions. You should consult an oral medicine specialist/dentist for examination, and if necessary, a biopsy. Meanwhile, keep good oral hygiene and avoid irritants like tobacco or alcohol

Black patches on the tongue without pain or other symptoms are often harmless. Causes include tongue pigmentation/black hairy tongue (due to dead cell buildup, smoking, coffee/tea, or medicines), benign pigmentation from drugs, or natural melanin deposits.
Next Steps
If patches increase, become painful, or you have risk factors (tobacco, alcohol), please consult a doctor (Dentist/ENT) for an examination. You may book an online consultation for personalized advice
Health Tips
Since you have no pain, ulcers, or swelling, it is usually not serious. Maintain good oral hygiene, brush/scrape the tongue daily, reduce coffee/tea and smoking if any, and stay hydrated. may book an online consultation for personalized advice.
