Binocular diplopia
I am suffering from binocular diplopia from one month.I am diabetic from 12 years and I am on insulin.My sugar level is in the range of 160-170. Blood pressure is normal,MRI is normal . Neurologist say it is a case of horizontal gaze palsy and some neurologist has prescribed me steroids and some neurologist has prescribed me other medicines like methcobalamin, tablet (KQ-100), nerve tonics. If I take steroids,my sugar level will raise. I have been taking nerve tonics and  methcobalamin,vitamins and other  fatty acid drugs but till now there is no sign of improvement.Please how long this diplopia will take or is it permanent?Help me find a solution for this . As you are diabetic , There are chances of getting Lateral rectus paresis . That gives diplopia in horizontal gaze . It tooks around 6 months for recovery . and steroids are not required . good glycemic control is required

Hello Your symptoms are suggestive of sixth nerve palsy. Risk factors include old age, systemic diseases like diabetes, hypertension etc As your MRI is normal... There is nothing to worry A.  Steroids are not required B.  Multivitamins are usually prescribed as happens in your case C. We observe for 6 months for spontaneous recovery D.  Prisms or surgery is required if there is no improvement even after 6 months Now what are options now: A.  Patch one eye to avoid diplopia B.  Intramuscular botox injection is an option now to temporarily get rid of diplopia Having said that it is provisional diagnosis Kindly share photos of your eye so that I may guide you better Regards

The commonest nerve affected is the 6 th nerve causing lateral gaze palsy and therefore Diplopoda. Steroids are not indicated . The vitamin B12 can be continued . Wear fogged or white glass over one eye to prevent Diplopoda from hampering your lifestyle. 90% resolve spontaneously in 4-6 months. Maskatieyeclinic Opposite Charni road station east, Mumbai
