First episode of prolapsed intervertebral disc revolves in 90% of cases with proper care and physiotherapy by 4-6 weeks. However there are three instances which require urgent attention with surgery if required. The first is any loss of muscle power. This is usually suggestive of a more severe compression and is unlike to improve fully by conservative means. The second is presence of any bladder symptoms like urgency or increase in frequency of urination. The third is neurogenic claudication, causing pain and heaviness in legs after standing or walking. This is usually marked by more of back pain than leg pain and may be due to spinal column instability.
