Patient Case Summary: May 16-18: Started MTP at 5 weeks pregnant (Mifepristone + 4 Misoprostol). May 27 Scan: Confirmed RPOC measuring 8.4mm * 6.7mm, minimal vascularity. May 27-29: Finished 2nd course of Misoprostol (6 pills). No bleeding yet, only back/leg pain. No fever. June 3 (Today): Had brief unprotected sex (no ejaculation, dry penis). Pee after and took i-Pill within 15 minutes.                                                            Note: I am a student and cannot afford private consultations or surgery. Please provide medical advice based on this.                                           Questions:                                       Is it normal to have no bleeding 5 days after the 2nd misoprostol course for a tiny 8.4mm RPOC? Can I expect it to flush out safely with my next period? When should I expect my next period to arrive after taking the emergency pill today, and on what exact date should I do a home urine pregnancy test to confirm I am not pregnant?

From your history, I don’t think you have gotten an ultrasound done before taking the first Mtp medicine. The RPOC on the second Mtp is definitely small, but usually not recommended to take take second set of Mtp medication. Nowhere recommended my advice to you is to have sexual responsibility and not to take your own health lightly. For now. You should get an ultrasound done to either check for the RPOC whether clear or not.

As no bleeding happened after repeat  dose means RPOC is still inside. Consult local gynecologist for surgical procedure.

Yes the patient may not bleed after the second dose of misoprostol Indicating failure of medical management This will require surgical evacuation as retain products are wonderful medium for infection infection if sets in can lead took tubal blockage and thereby  jeopardize future pregnancy prospect The date of next period is unpredictable
