HUGE FIBROID. Severe dysmenorrhea. Suffered from thigh pain & pelvic pain during period and upto ovulation. Felt relief only after ovulation and before next period. On ketorolac for whole month. What to do, pls advise. MRI with contrast report attached - 2 pages

Hi, Since your having adenomyosis. It's nature is to have unbearable pain which usually gets settled with analgesic. And your imaging studies shows the size is quite large. If you have completed your family (childrens) . It's good to go with surgery.

Its large adenomyoma , only treated with surgical management or can try mirena for an year , but if your family is completed do proceed with surgery  .

If family is complete , go for surgical management for adenomyoma . It is quite large and causing severe symptoms which needs management. Prolonged use of painkillers has many side effects.

I would advise mirena for some time and see if you respond, however in adnomyosis chances of expulsion of mirena are more. Hysterectomy is indeed the best treatment. However due to young age I would like to give mirena the benifit of doubt in your case
