You should get tested for thyroid hormones 2 monthly till 6 months of age and then every 3 months. So at this point of time get Thyroid function tests done - TSH alone is not enough, FT4 and T4 should also be done
It seems that the treatment was started without a chance to decide the cause suggestive of permanent hypothyroidism. We don’t see here a detailed report of investigations performed. We don’t want to stop thyronorm at this moment for fear of effect of hypothyroidism on neurocognitive functions.
If the new thyroid functions suggest that carrying on thyronorm was a logical solution we should continue thyronorm and titrate its dose as per the thyroid function reports
There is chance to re-evaluate at 3 years of age. The period for rapid brain development is over. And the thyronorm can be stopped for 4-6 weeks and thyroid function tested again. That’s the window to check for the justification of therapy
Next Steps
Do TSH, FT4 /T4. Plot the trend of hormones in the successive tests. Ensure compliance of treatment. On the day of test the baby should be given the thyronorm AFTER the test
Thyroid medication once started is generally continued for a very long time. However it is doubtful as to why the medication was started at a tsh of 10. Proper consultation with proper history of patient along with all reports is required. It's not a question that can be answered without all the details.
Next Steps
consult a paediatrician with all the reports for a second opinion.
