Since the skin folds are also involved, this looks more like a fungal (yeast) infection rather than simple diaper rash. Please use an antifungal cream like Candid or Silo-Derm c .

Likely started as a diaper rash and now appears fungal. Apply Candid ointment. Change diapers frequently. Keep the genital area clean and dry

Erythrmatous rash with Satellite lesions…looks like candidal infection…would suggest using candid-B ointment or Onitment Siloderm. Kindly Avoid using diapers
