"HbA1c 11.8 with high urine albumin
Hello Doctor, My father (age 66) is already diabetic and currently on diabetes medicines. His recent reports show: - Fasting glucose: 246 mg/dL - HbA1c: 11.8% - Urine Albumin (Immunoturbidimetry): 5450 mg/L - Albumin-Creatinine Ratio (ACR): 4242 mg/g (A3 category) - Urine protein: 3+ - Urine glucose: traces - Blood in urine: trace - Casts: Granular 1/8-10 LPF - eGFR: 91 - Creatinine: 0.92 mg/dL Kindly advise: 1. Do these results indicate diabetic kidney damage? 2. Are urine albumin and granular casts concerning? 3. What does trace blood in urine mean? 4. Should we change or intensify diabetes medicines? 5. Do we need to start insulin? 6. What diet restrictions should we follow now? 7. Any urgent additional tests required? Please guide on next steps.

Yes the result indicate kidney damage probably due to diabetes. Proportion of albumin, proteins and blod in urine also indicate kidney damage, concerning levels but do not indicate severity of disease. You will have to connect with nephrologist for further evaluation. You will have to modify diet and adjust medical OHA dosage, whether to add insulin or not is to be discussed with your endocrinologist.

Hi, Sugars are not in control.. he should start with insulin as u said he is already on oral medications His reports are really not satisfactory, and high chances of progressing to nephropathy if untreated, He need to be examined and have a regular follow up, so it's good if u consult a physician near by
